Use this form to choose between Annual Cost of Service or an alternative pricing plan. Open enrollment continues through 5 p.m., Nov. 24.
- Pricing Plan Selection
If you left the Annual Cost of Service rate in a previous month and are not with an ESS, you may fill out this form December 15-16 to switch between Daily or Monthly prices effective January 1, 2009.
